INSTRUCTIONS FOR COMPLETING CSE IEP AMENDMENT FORM
An amendment form is ready and attached for your use. It is very important that it be used in such a way as to insure the integrity of the CSE process. Please follow the directions stated below and contact me if you have any questions regarding the use of this form.
The amendment form is intended for use at the Elementary, Middle or High School. It is also intended only for changes of a minor nature and only for changes in which all school staff and the parents agree. For example, the additions of a mainstreamed class, the addition/deletion of a related service, the addition/deletion of a test modification, etc. This form would not be used to change a student from an out-of-district placement or for any changes about which the parents have concerns. Anytime we move a child to a more restrictive learning environment, a CSE meeting will be held.
Please complete the form as follows:
1Step 1- Teacher or related service provider completes student ID information and states rationale for change and lists the deletion or addition of IEP goals. Please be specific and state if you are deleting or adding IEP goals. Since parents don’t have a goal book, the narrative part of the change/rationale should be clear and specific.
2Step 2 – I will review the proposed amendment. If approved, it will be signed and a meeting date will be established. This information will be entered onto the form. If the request is not approved or if the form has not been completed correctly, it will be returned with an explanation or I will call you regarding my questions.
3Step 3 – The form is returned to the teacher or related service provider who then reviews the proposed change in depth with the parents and sends the form home to be signed. Parents MUST state if they approved of the proposed change and the MUST select whether or not they want to attend the CSE Sub Committee meeting. Finally, the parent MUST sign the form and date it.
4Step 4 – The teacher or related service provider receives the form from the parent and, after checking it for completeness, forwards it to the Pupil Personnel Services office.
Note: If the parent wants to attend the CSE Sub Committee meeting, the proposed changes will be reviewed at the date specified on the proposed amendment.
If the parent does not wish to attend the CSE Sub Committee, the amendment will be reviewed and completed at the next building CSE Sub Committee meeting.
It is therefore more expedient to process the amendment if the parent elects not to attend the CSE meeting. However, we must not discourage parents from attending the CSE meetings, if they feel they would like to attend.
I know the instructions seem long for a process that is supposed to lessen the time needed for minor changes, but I believe any matter related to the CSE process needs to be fully understood due to the legal nature of the CSE.
